Please note: If you were placed on a Wait List and there is a change in your address, or phone number you must notify us in writing as soon as possible. Please include your social security number and a copy of a photo ID with your correspondence. For convenience, you may use the Waiting List Update form provided below. ALL CHANGES MUST BE IN WRITING. No changes will be taken over the phone. If you do not respond to our notifications and your letter is returned undeliverable, your name will be removed from the Wait List.
