Elder Care Services

The Elder Care and Crisis Management department of the West Palm Beach Housing Authority currently serves 243 elderly residents that are 62 years of age and older. The department also provides crisis intervention assistance to public housing residents when necessary.

The WPBHA provides these residents with a Ph.D. level care manager so that medical, psychiatric, social and other needs can be met. The care manager works with an elder care assistant as well. The two work primarily in the field, not the office.

Below is an overview of the work done by this department.

   
Mental Health Consultations and Services

Educate residents about mental health treatment opportunities and mental health issues.

Communication with Medical Professionals

Ensuring residents obtain adequate treatment (mental and physical), obtaining approval for home nursing care, and obtaining approval for home psychiatric care

Hurricane Preparation

Home visits to ensure hurricane preparedness, registration of special needs seniors for special needs shelters

Social Security (SS) and Supplemental Security (SSI)

Visits to SS office with residents to sign them up for SSI and increase benefit amounts, online applications for “extra help” with Medicare D plans due to low income status

Arts and Enrichment

Arrange outings for residents to senior health fairs, movies, and performances.

Referrals For Residents Of All Ages

Information and referral for any number of issues including food banks, grandparent visitation issues, eviction assistance, complicated immigration issues, daycare, AIDS services in the community, counseling, exercise programs, nutrition, etc.

Adult Protective Services (APS)

When vulnerable adults are endangered or at risk, and WPBHA interventions are impossible or have proven inadequate, the state is brought in through APS reporting.

Medicaid

Enroll residents in Medicaid whenever possible, educate residents about Medicaid options including HMOs and Medipass, educate residents about Medicaid, assist residents in getting Medicaid eyeglasses, and apply for Medicaid Waiver for residents so they can me in nursing home diversion program (avoid nursing homes as long as possible)

Medicare

Enroll residents in Medicare whenever possible, educate residents about Medicare options including HMOs and straight Medicare, educate residents and pharmacies regarding low-income people and their Medicare-D benefits, and educate residents on Medicare issues.

Community Relations

Elder services makes an effort to maintain good relationships with the entire elder network in the city, county and state. We are constantly looking for innovative ways to meet the needs of our residents.

Health Care District (HCD)

Screen residents for Health Care District, Medicaid, and other programs more efficiently.

Consultations with PB County Division of Senior Services (DOSS)

Personal care, homemaking, respite and other services.

Food Stamps

Assist residents with activating food stamp (WBT) cards, choosing PIN numbers, replacing lost EBT cards, etc.

Legal Matters (non immigration)

Arrange health care surrogacies and powers of attorney for residents/family members when residents’ health or capacity begins to decline.

Transportation

Provide limited transportation for residents as well as apply for transportation assistance for residents.

Child Protective Services (CPS)

Make reports when a child has been harmed, neglected, or is in danger.

Immigration

Assist with renewing of residency status and naturalization applications. Bring in immigration lawyer to speak to residents annually, translate, and arrange/provide transportation.

Home Visits and Assessments

Evaluate condition and living situation of residents, review medication and medical insurance needs with residents. Assess social, psychosocial, emotional needs of residents.

Skilled Care

Make arrangements, with family if it exists, for nursing home care or hospitalization. Make nursing home and hospital visits as long as resident remains on lease.

Elder Care Services serves as assistance with obtaining important documents for immigration, recertification, food stamps, etc. such as birth certificates, driver’s licenses, divorce documentation. Guidance on hygiene including hair care, tooth care, bathing, etc. Outreach for donations to assist with unexpected resident expenses such as pet care, scooter batteries, diabetes shoes, etc.

WPBHA seniors remain living independently with a good quality of life for as long as possible, obtaining services such as home healthcare, alcoholism treatment, and more.
